Fees for non NHS GP Services

With effect from August 2020:

It is not possible to provide all services under the National Health Service and the following table therefore shows the current level of private fees, and the services for which you may have to pay.

Please remember, however, that all fees are charged at the discretion of the individual GP.

If you have any concerns about the level of fee involved, please discuss this with your doctor prior to requesting the service.

Private Medical Examination with report

HGV and PCV, (requires seperate eye test at an optician)	£120.00*
Pre-employment	£120.00*
Elderly driver fitness, racing driver, sports fitness	£120.00*
Taxi Driver	£120.00*

GP Letter/Report

Short (less than A4)	£40.00*
Standard	£96.00*
Lengthy (2 pages or more)	£132.00*

Certificate of Vaccination	£30.00*
Private Prescription	£15.00
Firearm application form	£90.00
General forms (i.e. Insurance/travel etc)	£30.00 - £63.00
Holiday Cancellation Certificate	£30.00 - £63.00
Private Consultation (per Hour)	£210.00
Childminder Forms	£36.00-£120.00*
Access to Medical Records/copies. (Subject Access Request)	No Charge
Council tax exemption form	No Charge

Please note the following:

Minimum charge is £5.00
Where there is a scale the cost will depend on the complexity of the form
Payment is required before the request can be sent to a GP.
Methods of payment are bank transfer (bank account details will be provided), cheque or cash.

* Items include VAT at standard rate of 20%
